Genoa Area Local Schools

The immunization requirements have changed for the 2012-2013 school year. Please refer to the list below.

 2012-2013 School Year

Kindergarten Entry

DTaP(or any combination)-4-5 doses  The 4th dose must be given after the 4th birthday or a 5th dose will be required.

Polio (any combination) 3-4 doses A final dose must be given after the 4th birthday or a booster will be needed regardless of number of previous doses

MMR -2 doses The first dose must be given after the first birthday

Hepatitis B- 3 doses timing is important here. If the timing is off, a booster will be required

Varicella (Chicken Pox)-2 doses prior to kdg entry or a signed note stating that the child has had the Chicken Pox

Also required for kindergarten entry are:

Birth certificate 
Social Security card 
Immunization record
Doctor's physical for kindergarten dated in current school year
Dental exam dated in current school year
Custody papers (if appropriate)

7th Grade Entry

TdaP (or any combination)-1 dose a booster is required prior to entry into 7th grade. This requirement will continue each year for 7th graders.
